Informed Consent *

You are being asked to participate in a research study to look at the possible relationship between regular physical exercise and affect, anxiety, depression and pain. If you agree to participate, you will be asked to complete a set of online surveys every month. This study has no known risks outside of those typical of regular physical exercise. In exchange for your participation, you will receive a free gym membership at participating Pivotal Fitness locations. You may experience health benefits by participating in regular physical exercise. You will also be making a contribution to the information known about the impact of exercise on affect, anxiety, depression and pain which may help prevent suicide and benefit others in the future. Your participation in this research is completely voluntary. You may also stop your participation at any time. If you do not want to participate or withdraw from the study early, there will be no penalty. This study seeks combat veterans. Your combat action will be verified via submission of a copy of your DD-214. You will not be eligible for this study or the free Pivotal membership without first submitting a copy of your DD214. Instructions for submitting your DD214 will be provided after you submit this form. Only Operation Vet Fit (OVF) will keep a record of all surveys in a secure database. Only the researcher will know the identity of study participants survey data. The surveys you complete in this study will not be distributed for any research outside of the agency. Only OVF will maintain your identifiable information such as name and contact information indefinitely and may contact you for further follow up research.
